What to Bring to Your First Appointment at Integrative Care of North Texas

Preparing for your first visit at our Frisco, Plano, or McKinney integrated medicine clinics helps us make the most of your time and ensures your care starts smoothly. Please review the list below and make sure you bring all the necessary items to your first appointment.

Be sure to bring:

Completed patient forms (you may also arrive 30 minutes early to complete them in person)
A valid photo ID
Your current insurance card
All medication bottles you are currently taking
Any relevant medical records, such as lab results, recent physicals, IQ or psychological testing results
For children: recent report cards or behavior sheets

Please note: New patients are required to arrive 30 minutes before their scheduled appointment to complete registration and check-in. This helps us verify your information, review your paperwork, and give you the full benefit of your appointment time.
