For your first visit, please bring a valid photo ID, your insurance card (both front and back), and a list of any current medications, vitamins, or supplements you’re taking. If you’re coming in for physical medicine services like chiropractic or physical therapy, wear comfortable clothing that allows for easy movement. If you have any relevant medical records, imaging results (X-rays, MRIs), or lab work from other providers, bringing those can be helpful, though not required.
